Patient Advocate
Facilitates the organization of improvement teams and participates in Patient Experience initiatives. Follows patients throughout the continuum of care, responding to patient and staff requests for intervention to resolve problems. Identifies and resolves problems which inhibit the quality of care and progression of the patient through the health care system. Identifies system deficiencies that cause problems and complaints. Facilitates the organization of improvement teams. Documents and maintains a tracking and trending system for problems and concerns identified by patients, families and staff. Identifies potentially litigious cases for referral. Communicates and partners with appropriate staff to identify and discuss areas of concern, problems and issues of relevance.
Bachelor's degree. One year experience in a patient care or healthcare environment. Must pass pre-employment skills test as required and administered by Human Resources.
